Spravato® (Esketamine) Treatment

Spravato® (esketamine) is an innovative, FDA-approved nasal spray treatment for adults battling severe forms of depression. It is specifically indicated for:
Treatment-Resistant Depression (TRD): For adults who have not
found adequate relief
from other traditional oral antidepressant medications.
Major Depressive Disorder (MDD) with Acute Suicidal Ideation or Behavior: To rapidly reduce depressive symptoms in patients experiencing a psychiatric emergency.
Unlike conventional antidepressants that primarily target serotonin or norepinephrine and can take several weeks to show effect, Spravato® targets a different brain pathway - the N- methyl-D-aspartate (NMDA) receptor. This distinct mechanism of action offers the potential for rapid improvement in symptoms, with some patients experiencing relief within hours to days.

Your Required First Step: Comprehensive Consultation
Before initiating Spravato® treatment, a thorough consultation is mandatory to ensure its safety and appropriateness for your specific condition. During this essential appointment, one of our licensed and experienced providers will:
Review Your Comprehensive Mental Health History:
Including previous diagnoses, treatment attempts, and responses
to prior therapies.
Conduct a Detailed Medical and Medication Evaluation:
To assess your overall health, identify any contraindications, and review
your current medication regimen.
Determine Clinical Candidacy: A personalized assessment
will be conducted to confirm whether Spravato® is a safe and
appropriate treatment option for you.
It's important to understand that not every patient is a candidate for Spravato® treatment. This comprehensive consultation is designed to ensure that your individualized care plan is effective, safe, and tailored to your unique needs.
What to Expect From the Spravato® Treatment Process
If deemed appropriate following your consultation, Spravato® is administered exclusively in our clinic under direct medical supervision, adhering strictly to FDA and REMS (Risk Evaluation and Mitigation Strategy) program requirements.
Each treatment visit typically includes:
In-Clinic Administration: You will self-administer the Spravato® nasal spray in a
comfortable, private setting within our office.
Mandatory Monitoring Period: Following administration, you will remain in the clinic for a minimum of two (2) hours for observation and safety monitoring by our trained healthcare professionals. This period is crucial for monitoring potential side effects such as dissociation or changes in blood pressure.
Ongoing Structured Care: Spravato® treatment follows a structured schedule, typically involving an induction phase (e.g., twice weekly) followed by a maintenance phase (e.g., weekly or bi-weekly), with regular check-ins to continuously assess your progress and adjust your care plan as needed.
Combination Therapy: As per FDA guidelines and clinical best practices, Spravato® is always used in conjunction with a daily oral antidepressant.
Important Note on Transportation: Due to potential side effects like sedation and
dissociation, you must arrange for a reliable adult to drive you home after each Spravato® treatment session. You should not drive or operate heavy machinery until the day after treatment, following a full night’s sleep.
